What To Have Ready.

Useful, not required.

Participant name, date of birth and NDIS number

How to reach you: name, role, organisation and phone

Forensic or justice involvement: order, bail, parole or youth justice

Primary disability

Plan management: plan managed, agency managed or self managed

Plan start and end dates, if you have them

Which supports are needed: SIL, daily living, community access, high intensity

Whether support coordination is also wanted

Known risks, behaviours of concern or authorised restrictive practices

Anything affecting who the participant can live with

Do not hold a referral back because something here is missing. Send what you have and we will work the rest out with you. Documents the form cannot hold — plans, behaviour support plans, order conditions — can be emailed to admin@fdsq.com.au with the participant's name.

What Happens Next.

Within two hours

We respond with a yes or a no, and if it is a yes, what we still need from you.

Within a day

We arrange a call, and where the participant is in custody or a forensic setting, we start organising the first in reach visit.

Within four days

Workers matched to the placement, and the roster built around the participant's actual obligations.
