Feedback and Complaints
If something is not right, we want to hear it. A complaint will never affect a person's supports, and you do not have to give your name.
How to make a complaint to FDSQ
Any of these is enough. Use whichever is easiest.

Through a family member, support person, advocate or support coordinator acting for you
You can make a complaint anonymously. We will still investigate it. If you do not give us contact details we cannot tell you the outcome, so tell us how to reach you if you want a response.
What happens, and how quickly
We acknowledge every complaint within two business days, in the way you asked to be contacted. We tell you who is handling it.
We aim to resolve complaints within 21 days. Where it will take longer, we tell you why and keep you updated at least every 14 days until it is closed.
You get the outcome in writing: what we found, what we are doing about it, and what to do if you are not satisfied. You can bring a support person or advocate to any part of this.
A complaint will not affect your supports
Making a complaint will never reduce, change or end a person's supports, and it will never affect how they are treated by FDSQ workers. Complaints are recorded, reviewed by management and used to change how we work.
You can go straight to the NDIS Commission
You do not have to come to FDSQ first. At any time, for any reason, you can complain directly to the NDIS Quality and Safeguards Commission.

Other places you can go
The Office of the Public Guardian, an independent advocacy service, ATSILS, or the Office of the Australian Information Commissioner for privacy matters. We will help you contact any of them if you ask.
