Privacy Policy

Forensic Disability Supports Queensland (FDSQ) handles personal information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles. This policy explains what we collect, why, who we share it with, how we store it, and how you can access or correct it.
Information we collect
We collect personal information necessary to assess a referral and deliver supports safely. This includes name and contact details, NDIS number and plan details, funding and plan management arrangements, and details of the supports requested.
We also collect sensitive information, including health information, disability and diagnostic information, medication and clinical information, behaviour support plans and authorised restrictive practices, risk and offending history, order and condition details, and information about cultural identity where a participant chooses to provide it.
Information received from third parties
A large share of the information we hold reaches us from someone other than the participant. Support coordinators and specialist support coordinators, Queensland Corrective Services and Youth Justice, hospital and forensic mental health treating teams, behaviour support practitioners, the Office of the Public Guardian, legal representatives and family members all provide information as part of a referral. Where we receive information about a person from a third party, we take reasonable steps to make that person aware that we hold it and why.
Why we collect it
To assess whether we can support a person safely, to plan and deliver supports, to staff and roster appropriately, to meet our obligations as an NDIS registered provider, to record and report incidents and restrictive practices, to communicate with the people responsible for an order or a treating plan, and to invoice for supports delivered.
Who we disclose it to
We disclose personal and health information only where it is necessary and permitted. That includes the participant's support coordinator, behaviour support practitioner and treating team, the NDIS and the NDIS Quality and Safeguards Commission, plan managers for invoicing, and Queensland Corrective Services, Youth Justice or a court where an order or condition requires it.
We may also disclose information without consent where it is required by law, or where it is necessary to lessen or prevent a serious and imminent threat to the life, health or safety of any person.
Storage and security
Records are held in access controlled systems, with access limited to the workers and managers who need it for the support being delivered. Paper records, where they exist, are held securely. We retain records for the periods required by NDIS practice standards and Queensland law, and destroy or de-identify them when no longer required.
Access and correction
You can ask for the personal information we hold about you, and you can ask us to correct it if it is wrong or out of date. Email admin@fdsq.com.au or call 0439 143 082. We will respond within 30 days. If we refuse access or correction, we will tell you why in writing and how to have that decision reviewed.
Complaints about privacy
Contact us first at admin@fdsq.com.au. If you are not satisfied with our response, you can complain to the Office of the Australian Information Commissioner at oaic.gov.au or on 1300 363 992. You can also complain to the NDIS Quality and Safeguards Commission on 1800 035 544 at any time.
This website
The referral form on this site is provided by Astalty, the client management system we use to run supports. When you submit the form, the information you enter is sent to and stored in Astalty on our behalf, and our team is notified. The form is displayed on our pages from Astalty's servers, which means Astalty receives your IP address and basic device information when the page loads, and it uses Google reCAPTCHA and Google Maps for spam protection and address lookup. You can bypass the form entirely by calling or emailing us.
We use Vercel Web Analytics to understand how the site is used. It records page views and events such as a call button being tapped, without cookies and without building a profile of you. We do not run advertising pixels, marketing trackers or chat widgets, and we do not collect newsletter subscriptions.
Our website is hosted by Vercel, which keeps standard server logs. Information submitted through the referral form is used only to respond to that referral.
